More Support for People with Dementia in Lewisham

Working with South London and Maudsley NHS Foundation Trust and Lewisham Social Services, Lewisham MindCare Dementia Support has a new Community Support Worker to help people diagnosed with dementia attend social activities in the local community, attend medical appointments and provide practical support in the home (organising paperwork, using household appliances).

In the first few months of work, the Community Support Worker has helped improve the health and overall wellbeing of several clients living with dementia. The Community Support Worker helped one particular client attend GP and other health appointments, resulting in unnecessary medication being removed from their care plan and a long term health complaint being resolved.

Support from the Community Support Worker is practical and short term.
