Lewisham Dementia Information and Support Service provided support to 331 Lewisham residents with dementia between April 2016 and March 2017.
The team work very closely with the Lewisham Integrated Memory Service to deliver a joined up and comprehensive service to people from the point of diagnosis until end of life. Elements of the service include signposting, developing person-centred support plans, facilitating access to community resources and advocacy.
There was a significant increase in the number of individual support plan goals set and reviewed during the year, with a total of 72% of the 689 goals reviewed during the year being achieved.
During the year, funding was secured to provide a Dance for Dementia group in Catford, and an average of 8 people have attended each of the fortnightly sessions. We have also continued to provide the popular Exercise Group and Advocacy Group.
The Lewisham MindCare team continued to play a key role in the development of the Lewisham Dementia Action Alliance work to make the borough a dementia friendly community.
